Suicide Prevention Outreach Program for Transitional Aged Youth

Suicide is the third leading cause of death for youth 15 to 24 years old. 

Supporting Community Connections, or SCC is a suicide prevention outreach program for transitional-aged youth (TAY) in the foster, homeless, and LGBTQ communities. The program seeks to prevent suicide by offering connections and links to housing opportunities, mental health services, women’s health providers, LGBTQ+ resources, and other social services to transition-aged youth.

Serving Transition Aged Youth (TAY) LGBTQ+, Foster, and Homeless Youth

Supporting Community Connections, or SCC is a grant-funded program that was developed based on the County of Sacramento identifying the need for prevention and early intervention services for transition-age youth (ages 12-25).

SCC offers educational groups on the Children’s Receiving Home campus and in the community that teaches youth how to use resources to promote health and well-being. Individual case management and peer support are also provided by SCC staff to both TAY in the community and on campus as needed to facilitate linkages, connecting homeless and runaway youth with community resources and emergency supplies. Outreach to TAY is conducted through multiple events throughout the year including Pride, Suicide Prevention Walks, and local wellness fairs.

Groups Include

  • SuicideTALK workshops for suicide prevention open to TAY in the community and on campus, as well as to staff and community partners
  • Life Skills Groups for adolescent foster youth on campus as well as foster youth and homeless youth in the community
  • Adolescent Social Skills and Self Advocacy groups open to adolescent foster youth on campus
  • LGBTQ Teen Support group which is open to TAY in the community and on campus
  • Seeking Safety Evidence Based Practice curriculum for trauma and substance use which is open to adolescent foster youth on campus



The overall goal for SCC is to provide these resources to help youth feel healthy, empowered , and connected.
